The Federal Drug Administration on Friday issued a warning to doctors that adolescents taking the drug olanzapine have an "increased potential" — in comparison with adults taking the new-generation antipsychotic drug — for weight gain and metabolic disturbances that could result in diabetes or elevated blood cholesterol levels.

"Clinicians should consider the potential long-term risks when prescribing to adolescents," the FDA said in a statement released Friday night. "In many cases, this may lead to prescribe other drugs first," the statement went on.

The agency has not approved the marketing of olanzapine — sold under the commercial name Zyprexa by the drug maker Eli Lilly — for use in children under 13 who are diagnosed with schizophrenia or bipolar disorder. But the medication, one of a class of psychiatric medications called the "atypical antipsychotics," is widely prescribed for young patients, despite growing evidence that call its safety profile into question for this population.

The warning comes in the wake of the October publication in the Journal of the American Medical Assn., in which a study showed that children and adolescents taking their first-ever course of Zyprexa gained, on average, more than 17 pounds over a 12-week period, as well as dramatic increases in triglycerides and cholesterol levels — all factors that put them at higher risk of developing cardiovascular disease. While two other antipsychotic drugs — Seroquel and Risperdal — were implicated in significant weight gain and metabolic changes, Zyprexa was found to carry the highest risks of all three.  

The FDA said that in cases where physicians opt to prescribe olanzapine, the drug should be part of a "comprehensive treatment program," which could include psychological, educational and social counseling as well as medication therapy.

Johnson & Johnson (JNJ) shares are rebounding today, up nearly 1.5% to $65.51. The drugmaker is trying to recover from a rough week, when the Justice Department charged it with paying kickbacks to a pharmacy company to boost sales of its drugs to nursing-home patients, and the Food and Drug Administration accused it of being too slow with its product recall during a Tylenol scare.

Late last year, J&J, based in New Brunswick, N.J., expanded its recall of its top-selling Tylenol line, which sold at least $1 billion in the U.S. last year. The incident echoes an unsolved 1982 crime in which seven Chicago-area Tylenol consumers died after using tampered Tylenol laced with cyanide. Johnson & Johnson's quick, decisive response to the incident earned plaudits and sparked reforms in packaging over-the-counter drugs. The current recalls began in September, expanded in late December, and then expanded again last week to include more batches and other brands.

Voluntary Recall

The issue at hand is an unusual moldy, musty, or mildewy odor in the drugs caused by small amounts of a chemical associated with the treatment of wooden pallets. Users who reported the smell complained of nausea, stomach pain, vomiting, and diarrhea.

Despite a voluntary recall, J&J's McNeil Consumer Healthcare Products got a stern warning from the FDA for manufacturing-standards violations and for failing to report and investigate the problem quickly. Timothy Calkins, a marketing professor at Northwestern University's Kellogg School of Management, told The New York Times that the FDA's criticisms “were devastating, because they make the company seem to be complacent and sloppy.”

To make matters worse, the Justice Department has filed a civil False Claims Act complaint against Johnson & Johnson for paying millions of dollars in kickbacks to Omnicare, the nation's largest pharmacy to specialize in dispensing drugs to nursing homes.

The allegation isn't a huge surprise: In Novevember, Omnicare entered into a $98 million settlement for taking kickbacks from J&J. Omnicare's pharmacists, according to the complaint, reviewed charts of nursing-home patients with dementia and then recommended that the physicians add antipsychotic drug Risperdal if the patient showed signs of agitation or inappropriate behavior. Physicians, who were not involved in the scheme, agreed with the pharmacists' recommendations about 80% of the time, although Risperdal isn't approved to treat this form of psychosis.

“Kickbacks in the nursing home pharmacy context are particularly nefarious, because they can result in excessive prescribing of strong drugs to patients who have little or no control over the medical care they are receiving,” said U.S. Attorney Carmen Ortiz.

Risperdal sales brought J&J $1.73 billion in the first nine months of 2009: nearly 4% of the company's $45.3 billion in revenue.

Chemical Restraints

Risperdal doesn't reverse or relieve dementia, and in fact carries a “black box” warning of increased chances of death among elderly patients with dementia-related psychosis. Still, it has been widely prescribed in nursing homes, in a practice criticized as “chemical restraints”: altering patients' behavior for the convenience of the staff. The practice still continued by 2006, as a study published Monday in the Archives of Internal Medicine shows, although at a lower rate. Because most elderly patients are covered by Medicaid, the lawsuit suggests, J&J and Omnicare J&J and Omnicare had to have been creative in accounting for the kickbacks and listed them as education funding, grants, and data on the drug. J&J did not respond to inquiries.

J&J is on the hook not only with the FDA and the DOJ but with public opinion. The company's actions after the Tylenol scare of 1982 fully restored its corporate and brand images, but this recall and the FDA letter (not to mention the kickbacks allegations) has harmed its stature.

J&J reports financial results on Jan. 26. Whether the recall affects its business is unclear. The company doesn't expect the recall to be material, but the lasting effects on the company's image might be significant.

I grew up in Chingford in the sixties and seventies. Although Chingford has for many years been part of the East London borough of Waltham Forest, it is in fact a leafy green suburb on the border of Epping Forest, Essex. It was once quite rural but started to become more urban during the Second World War, when inhabitants of the East End of London started moving there to avoid the bombings. In fact, one of my earliest memories is of going grocery shopping with my mother and bumping into previous neighbours from her own childhood in Hackney.

Just around the corner from our house was Pole Hill, an entry point to Epping Forest. My friends and I would often play hide and seek there, free from adult supervision. There was an obelisk landmark at the top of the hill which is still there, though it's often covered in graffiti nowadays. Cattle used to graze on the forest land nearby and I can recall sitting in our dining room and watching them munch their way through out front garden. Today, they wouldn't be able to squeeze between the tightly parked cars!

Some of my happiest childhood memories are of spending long, lazy afternoons at Larkswood open air swimming pool with my mum, younger brother and an assortment of friends. It's where I learnt to swim. Sadly, it closed many years ago and remained derelict for a while. It's now a leisure centre.

Chingford High Street (Station Road) was typical of many of that era, before giant supermarkets existed. There were family-run bakeries, butchers and green grocers, as well as a fishmonger. The shop I remember most vividly was a sweet shop called Hargreaves, run by two elderly sisters. Its shelves were crammed full of large jars of sweets which you could purchase by weight. Lemon sherbet lollies and gobstoppers were among my favourites. There was a newsagents called Brimbles, a haberdashery shop called Bassets, run by a lady named Winnie Basset, who seemed to stock anything and everything. There were also three small grocery stores, Chasneys, Stitchers and the Co-op (which is still there today).

At one end of Station Road is Chingford Green, which houses the library and St. Peter and St. Paul Parish Church. The BBC programme, Songs of Praise, was filmed there during my Brownie days, although the only thing I remember about it was the cameraman, who was positioned right in front of our Brownie pew. My friend and I took it in turns to pinch his bottom throughout the service. We thought it was a hoot, though we must only have been about seven or eight years old at the time!

I have visited Chingford often over the years as my parents still live in the house where I grew up. Thankfully, it has retained much of its character, although there have been inevitable changes. Some shops have been replaced by restaurants or bars and others have become supermarket chains but a few are still thriving after more than 50 years in business.

Having lived abroad and in other parts of London I recently moved back to Chingford. It still feels like home.

10,000 Steps: Walking Your Way To Health and Fitness

It can't be that simple! Everyone knows that Health and Fitness equals Exercise and Exercise is hard work! It requires special clothes, complicated formulas involving heart rates and body mass index's, expensive equipment, sweat and most of all, pain. Lots of it. It couldn't be as simple as… walking. It is!

We've all heard how sedentary our lives have become but have you really thought about how little you move in a day? How often do you take the stairs or park a few spaces further away from the entrance to the mall? Once you arrive at work, do you have to leave your cubicle/office at all? If you do, how far is the cafeteria anyway? That's why gaining fitness is as easy as doing a little more of something you've done since you were a baby.

So what is the 10,000 steps about? It is the number of steps per day the average person is supposed to need for improved health and fitness. It equates to about five miles. Do not panic! That is the amount for the average person. If you are only walking 1000 steps per day, moving up to 1500 steps per day will be more than enough for awhile.

You do not have to do all of that walking at once. This can be broken up into several short sessions throughout the day. You should also look for opportunities to add steps. Walk around the office while you are on a call. Park further from entrances. Go to the mall and walk around an entire level (just don't take your checkbook with you or an inexpensive fitness routine may be come very costly!).

For those of you who must have an equipment list, charts, graphs and calculations, here you go.

Equipment required:
One good pair of walking shoes
(Two pair allows for rotation, extending wear time and improving foot health.)
A pedometer
Clothing suitable for the weather conditions
Some place to walk.

Your shoes should fit well, particularly at the heel, provide sufficient arch support and allow the expansion of the foot that is the natural result of walking.
The pedometer can be a simple step counter without the mileage conversion. They come in analog and digital models.
The places to walk can be the mall, a local track, your office building (a good place to build a walking club) or around your neighborhood.
Walking buddies are optional but add to the fun!

The following chart shows how many calories are burned per hour of walking based on current weight and speed walked. To calculate the rate of speed, you need a known distance such as a walking track and a watch. If no track is available, get in your car (yes, I said get in your car) and drive the route you plan to walk, measuring the distance. Then get out of the car (stop groaning) and walk the course with your watch. If you walked a mile in fifteen minutes, you'd be walking at a rate of four miles per hour (if you need a formula, go check out some of the excellent books and internet sources on walking).

Some walking tips:

Stand up straight, shoulders relaxed, arms hanging loosely at your sides. Allow your arms to move naturally with your body as you walk. Don't swing your arms across the front of your body. This is wasted movement and can cause injury because it causes the shoulders to tense. Keep your head up and eyes forward. This ensures proper posture and makes walking a comfortable exercise.

Do not use weights. They put strain on your joints and can throw off your posture, resulting in pain. This is not supposed to be painful!

If you feel you need a heavier workout, walk faster! Just maintain your proper walking posture but try to cover your previous distance in a shorter amount of time. Do not overstride. Take shorter, quicker steps.

Drink water. Extra exertion requires extra hydration.

Dress appropriately for the weather and pay attention to air quality alerts if you have an outdoor walking program. You are walking to improve your health and quality of life. Neither are possible if you are sucking in smog or get frostbite!

Do stretch after walking. This allows for proper cool down of the muscles, improves flexibility and prevents injury.

Start gradually. If you normally log 2000 steps per day, add 500 more. Some studies have shown that adding a few extra steps per day can assist in preventing weight gain.

Finally, pay attention to your body. Some signs of overtraining are excessive tiredness, very sore muscles or catching a cold. It is okay to skip a day or two (only a day or two).

Walking. You've been doing it for such a long time. Who knew it was actually good for you?! ;-)

There is never a good time to be without health insurance. One medical emergency can destroy years of hard work. If you objectively analyze your risk, most people are able to find an insurance policy within their budget.

I permanently consider that Little kids have to be active every day. Physical activity stimulates growth and leads to amelioration physical and emotional health. Nowadays, research tell to us that the importance of sports in kids is stronger than ever. For example, medical investigator have looked that highly active children are less likely to suffer from obesity later in life. Sport is also known to relieve stress. Some children experience as much stress, and anxiety as adults do. And because sport increase health, a fit kid is more likely to be well-rested and mentally sharp.

Even moderate sport has been shown to improve a child's skill at arithmetic. The important goal of parents and trainers is to help children find the happy in exercise people need to make them feel valued and needed. Every child can be happy at one sport or another. Take the time to find the sports that are right for each child. Exercises is an significant learning environment for kids. One way children get acceptance by their peers is to be good at activities valued by other children.
